Purpose and Aim

To develop the skills of healthcare and social care support workers supporting home enteral tube feeding and care in community based settings. The unit complies with the All Wales Competency Framework to Support Adults who require Home Enteral Tube Feeding (HETF) via a Gastrostomy Feeding Tube.

1.1Outline the current national policy framework for HETF in community based settings in Wales.
1.2 Outline the current organisational policies for working with individuals who require HETF.
2. Understand why individuals require HETF.
2.1Explain the reasons for HETF.
3. Understand how HETF is performed.
3.1Identify the equipment used in enteral feeding.
3.2 Explain HETF processes.
3.3 Explain the administration of feed and water: 
  • via a gastrostomy tube
  • via a side port.
3.4 Explain the administration of medicine:
  • via a gastrostomy tube
  • via a side port.
3.5 Identify risks associated with HETF.
3.6 Identify responsibilities for risk assessment before HETF is performed.
3.7 Describe how to troubleshoot difficulties with HETF.
4. Know how to undertake the daily care of a gastrostomy tube.
4.1Explain the daily care of:
  • a percutaneous endoscopic gastrostomy (PEG) tube
  • a balloon gastrostomy
  • a button gastrostomy tube
  • a radiologically inserted gastrostomy (RIG) tube.
5. Be able to perform daily care of a gastrostomy.
5.1Provide daily gastrostomy care for an individual.
5.2 Identify how to seek support for complications with daily care of a gastrostomy.
6. Be able to support an individual to prepare for with HETF.
6.1Explain an individual’s care management plan for HETF.
6.2 Work with an individual to prepare for HETF.

7.1Provide enteral tube feeding for an individual at home.

reasons for HETF must include examples from each of the following categories:
  • acquired brain injury

prepare for HETF must include:
  • explaining the  HETF procedure to an individual
  • obtaining consent from the individual or others on his/her behalf, for the procedure
  • performing hand hygiene as defined in current NICE guidance ‘Prevention of healthcare-associated infection in primary and community care’ CG139
  • preparing the equipment:  feeding pump, giving set, feed containers, syringes
  • setting-up an enteral feed
  • positioning
  • protection of an individual’s clothing
  • supporting dignity.
enteral tube feeding (at home) must include:
  • following a care management plan
  • opening a gastrostomy tube
  • administering water
  • administering medication
  • administering enteral food
  • closing a gastrostomy tube
  • safely disposing of equipment: feeding pump, giving set, feed containers, syringes
  • recording the enteral feeding intervention.
