Purpose and Aim

This unit will be aimed at those who provide care to individuals with bowel/bladder stoma. This may be for individuals with new stomas or for individuals with established stomas who are unable to manage their own stoma care.
This unit is applicable in a variety of care environments, including hospitals, care homes, the individual's own home or other community environments such as GP surgeries.

1. Understand the governance of stoma care in Wales.
1.1Summarise the current legislation national guidelines, policies, protocols and good practice for undertaking stoma care in Wales.
1.2 Explain own responsibilities and accountability in relation to current legislation, national guidelines, organisational policies and protocols which affect work practice when carrying out stoma care.
2. Understand the anatomy and physiology for conditions requiring stoma care.
2.1 Explain the reasons why stomas are fashioned.
2.2 Describe the structure and function of:
  • colostomies
  • ileostomies
  • ileal conduits
  • nephrostomy.
 
2.3 Explain the potential consequences of contamination of stoma drainage systems.
2.4 Explain the effects of diet and mobility on stoma function.
3. Be able to prepare individuals for stoma care.
3.1 Confirm the individual’s identity and gain valid consent.
3.2 Communicate information in a way that is sensitive to the personal beliefs and preferences of the individual.
3.3 Carry out preparatory checks against the individual’s care plan.
3.4 Use standard precautions for infection prevention and control.
3.5 Apply health and safety measures relevant to the procedure and environment.
3.6 Prepare equipment and materials for stoma care as prescribed/detailed in the individual’s plan of care.
4. Be able to carry out stoma care with an individual.
4.1 Describe the factors which will affect the level of assistance required when working with an individual with a stoma.
4.2 Agree the level of support required with an individual.
4.3 Carry out stoma care in line with local policies and protocols.
4.4 Explain the action to take if the prepared stoma care equipment is inappropriate or unsuitable.
4.5 Assist individuals to select and consume food and drink.
4.6 Support individuals to dispose of their own used stoma care equipment.
4.7 Support individuals to maintain personal hygiene with a stoma.
4.8 Support individuals to manage their own stoma.
4.9 Dispose of equipment and soiled linen in line with local policy and protocol.
5. Be able to monitor and check an individual’s condition following stoma care.
5.1Monitor an  individual’s condition following the stoma care to include:
  • checking  for  adverse effects and complications
  • monitoring  and recording  an individual’s pattern of stoma function, consistency of body waste
  • monitoring  changes in the stoma
  • reporting findings to colleagues in line with local policy and protocol.
